FYHA 2023/24 Member Survey

Hello FYHA members,
 
Please assist us by completing the following survey on your family's experience with the Association this past season.  Your feedback is sent directly to the Board and will be used to make adjustments that improve your minor hockey experience next season. This 6-8min survey and its results are part of the Board's commitment to Members stated in Article 2.02.1: 
 - "consulting with and involving Members in decision-making" and
- "maintaining a safe and welcoming place for Member feedback".

Your email is required to ensure you are a Member in good standing with the Association and eligible to provide feedback. Individual responses are not identifiable or shared without your consent.  One response per qualified email per child is permitted.
